registration for June 10
MADISON — Preschool registration and screening will be held Tuesday, June 10, at Madison Elementary School on by ointment for students who reside in Madison.
If your child will be four years old on or before Oct. 15, 2014, call Madison Elementary School at 696-4607. Children will be registered and screened by appointment only. There is limited space in this program so call as soon as possible.
Bring your child, his/her birth certificate, current immunization records, and a copy of the current three- or four-year-old physical to the registration appointment.
The school board of School Administrative District 59 has adopted a policy that requires all children be up to date with immunizations including the varicella vaccine (chicken pox) and physical exams. These up to date records must be on file with the school nurse by the first day of the school year.
The Preschool program is currently running two sessions, morning and afternoon, Monday through Friday. The morning sessions run 8-10:30 a.m., the afternoon session runs 11:30 a.m.-2 p.m. Children must be transported to and from school.
