MADISON — Kindergarten registration and screening will be held at Madison Elementary School Wednesday, April 16, and Thursday, April 17, by appointment for students who reside in Madison.

If your child will be five years old on or before Oct. 15, call the school at 696-4607. Children will be registered and screened by appointment only. Call now or stop by the office.

Bring your child’s certified birth certificate, current immunization records, and a copy of current four- or five-year-old physical to the registration appointment. The school board of M-SAD 59 has adopted a policy that requires all children be up to date with immunizations including the varicella vaccine (chicken pox) and physical exams. These up-to-date records must be on file with the school nurse by the first day of the 2014-15 school year.

A family handbook containing information regarding kindergarten screening and programming is available in the Madison Elementary School office, pick one up prior to the registration appointment.
