MADISON — Kindergarten registration and screening will be Wednesday, April 11, and Thursday, April 12, at the Madison Elementary School. Madison children, who will be five years old on or before Oct. 15, will be registered and screened by appointment only. Call Madison Elementary School at 696-4607 for an appointment.

Parents/guardians need to bring the child’s birth certificate, current immunization records and a copy of a current four- or five-year-old physical to the registration appointment.

The Maine School Administrative District 59 school board adopted a policy that requires all children be current with immunizations including varicella vaccine (chicken pox) and physical exams. These up-to-date records must be on file with the school nurse by the first day of the 2012-13 school year.

A Family Handbook containing information about the screening process is available at the Madison Elementary School office. Parents are encouraged to pick one up prior to registration appointment.
