FARMINGTON — Franklin Health primary care medical offices have begun implementation of a patient-centered medical home model, a health-care setting that facilitates partnerships among patients, their team of health care providers, and, when appropriate, patients’ families.

The model designates a primary nurse-provider team that coordinates patient care across a continuum of settings ranging from the primary care office to specialty clinics to inpatient areas.

Care is facilitated by registries, information technology, health information exchange, and other means to assure patients receive the indicated care when and where they need it, and communicated in a way they understand.

A few employees who are patients at a Franklin Health primary care medical practice (Farmington Family Practice, Wilton Family Practice, Livermore Falls Family Practice, Pediatrics, and Internal Medicine) are piloting a patient portal that provides access to their health information via a secure Internet connection. Through this patient portal, patients can review their test results, office visit summaries and medication list; and they can request a prescription refill and an appointment.

It is expected that once the pilot and testing stage is complete, access to the patient portal will be extended to all Franklin Health primary care patients with computer access in January 2013.
