MADISON — Madison Elementary School will conduct preschool registration and screening for students who reside in Madison on Tuesday, June 4.

Children who will be 4 years old on before Oct. 15 are eligible. Children will be registered and screened by appointment only. Limited space is available. Parents or guardians are asked to bring student’s birth certificate, current immunization records and a copy of current physical exam to the appointment.

The Maine School Administrative District 59 school board has adopted a policy that requires all children be up to date with immunizations including the varicella vaccine (chicken pox) and physical exams, according to a news release. These updated records must be on file with the school nurse by the first day of the school year.

The preschool program is running two sessions: 8-10:30 a.m. and 11:30-2 p.m., both sessions are Monday through Friday. Students must be transported to and from school.
