MADISON — Kindergarten registration and screening will be held at Madison Elementary School April 13 and 14 by appointment for students who reside in Madison and Starks.

If your child will be 5 years old on or before Oct. 15, 2011, call Madison Elementary School at 696-4607. Children will be registered and screened by appointment only.

You need to bring your child, his/her birth certificate, current immunization records, and a copy of a current four- or five-year-old physical, to the registration appointment.

The school board of School Administrative District 59 has adopted a policy that requires all children be up-to-date with immunizations, including the vericella vaccine (chicken pox) and physical exams. These up-to-date records must be on file with the school nurse by the first day of the 2011-12 school year.

A family handbook containing information regarding kindergarten screening and programming is available in the Madison Elementary School office. It’s encouraged that parents pick one up prior to your child’s registration appointment.

There will be a parent information night at 6:30 p.m. April 6 at Madison Elementary School for parents of children attending kindergarten. The kindergarten staff will talk about the screening process, the kindergarten program and answer parent questions.
